FormW-2, Wage and Tax Statement

喊棗喝娶泭2022FormW-2is now availableif you elected to receive the form electronically. To access your2022Electronic FormW-2:

  • Login to
  • Go to Personalized Links, click Work @ 51勛圖
  • Click 兜-2泭釦喧硃喧梗鳥梗紳喧莽, log into Eagle Service, and select year 2022.

Your FormW-2will load in your web browser as a PDF file. If the file does not display correctly, try using a different browser such as Apple Safari or Google Chrome.

If you opted not to receive your FormW-2electronically or did not complete the consent formbefore January 17, a paper version will be mailed to your home address and postmarked by January 24.

If you would like to change the method by which you receive your FormW-2infutureyears:

  • Login to
  • Go toPersonalized Links, click Work@ 51勛圖
  • Click on 兜-2泭釦喧硃喧梗鳥梗紳喧莽 and log into Eagle Service
  • Click on Receive my W-2 only in electronic format and hit Save.

Form1095-C, Employer-Provided Health Insurance Offer and Coverage

In addition to theW-2, faculty and staff who were eligible for 51勛圖-sponsored medical coverage at any time during calendar year 2022 will receive aForm 1095-Cmailed to their home by the end of February. The form is required by the Affordable Care Act (ACA) and provides documentation to support your income tax filing for 2022. Kaiser medical plan participants also will receive a Form 1095-B directly from Kaiser.
